Thank you for choosing James A. Striebel Dentistry! Prior to your first appointment, we invite you to print and fill out the new patient forms below, which will make your check-in process a breeze. If you opt to not print them out prior to your visit, make sure to arrive at your appointment at least 15 minutes early to allow time to fill them out.

Financial Information

We do work with dental insurance and will file claims on your behalf. We accept cash, check, American Express, Discover, MasterCard, and Visa, as well as CareCredit and Lending Club for third-party financing and flexible payment plans.
